Reminder : COVID-19 Third Vaccine Dose at SIT

SIT will be offering additional dose of the COVID-19 vaccine (3rd dose) to SIT students, faculty and staff members.
Venue: Toyosu and Omiya Campus
Date: March 14,15,16,18
Vaccine: Takeda/Moderna
・You must have completed your 2nd dose of vaccination by August 18, 2021.
・You can get vaccinated by appointment only.
Appointment booking period January 25 - February 13 17:00.
・Please make an appointment on booking website via Scomb on after January 25.
・You can change or cancel your appointment during the booking period. However, after the period, you CANNOT cancel it.
・If you wish to change the date of your vaccination, you can do so from the website by the day before.
・If you have completed your 2nd vaccination by August 18 2021, you can apply even if you have not had your 1st or 2nd vaccination at SIT.
・Even if you don't have the additional vaccination ticket, you can apply for it as long as you can prove that you have received two doses of vaccination on the day of the additional vaccination.
・If you have had a relatively severe adverse reaction to a previous vaccination, or if you have a pre-existing medical condition, please consult your doctor. Undergraduate students and master's students should discuss about it with their parents or guardian before.
